Home-services information and assessments do not replace a clinical evaluation of medical or mobility needs. Funding agencies and program administrators determine eligibility and approval for their programs. Requests and Appointments
Sending a contact form starts a conversation; it does not book an appointment. When scheduling online, review the visit type, address, reserved time, and confirmation before relying on the booking. If real-time booking is unavailable, we contact you to confirm the appointment. Service availability and any travel terms are confirmed for your address.

Written Scope and Service Terms
Your written quote or service agreement identifies the work, the responsible provider, price, schedule, payment terms, and applicable warranty terms. Trade Partners
Live Oak handles home-accessibility installations and routine upkeep within its direct scope. For plumbing, electrical, tile, structural, and other work assigned to a trade partner, the homeowner contracts with the appropriate provider separately. We help define the scope and coordinate the work.
The written scope identifies who performs each part of the project and who handles any required permits and inspections. Live Oak work and separately contracted partner work have their own responsibilities and warranty terms. Ask us to clarify these before work begins.
